With the demand to deliver more patient-facing services, community pharmacy is becoming increasingly automated when it comes to dispensing. Hub and spoke technology, robotic dispensing, intelligent PMR systems and other digital technologies are transforming the way medicines are managed and dispensed, helping pharmacy teams improve efficiency while freeing up valuable time for patient-facing care.
But there is a fundamental dependency behind much of this technology: the barcode on the medicine pack must be accurate, complete and scannable.
When it is, the barcode becomes much more than a tool for identifying stock, it becomes part of the digital safety net that helps ensure the right dose of the right medicine reaches the right patient. When it isn't, the benefits of automation can begin to unravel.
A new GS1 UK survey, conducted between March and April 2026 among 44 secondary care organisations, provides a timely indication of how widespread barcode problems have become.

The survey also identified three recurring consequences: patient safety risks associated with workarounds, operational inefficiencies affecting robotic dispensing and stock management and barriers to implementing end-to-end closed-loop medicines management. Organisations using dispensing technology reported greater impact, reflecting just how dependent modern pharmacy technology is on reliable barcode data.
These findings should be a wake-up call. Barcode accuracy is not simply a supply-chain issue. It is a dispensing issue, an efficiency issue and, ultimately, a patient safety issue.
The purpose of pharmacy automation is to make dispensing processes safer and more efficient.
A dispensing system can use a barcode to identify a product quickly and consistently, compare it against the medication that has been prescribed and provide an additional check before the medicine reaches the patient.
That is particularly important as pharmacies increasingly adopt technologies such as hub-and-spoke models and intelligent PMR systems. But these systems cannot compensate for bad product data. A missing barcode stops the automated process and an unscannable barcode stops the automated process.
An incorrect barcode can be even more concerning because it has the potential to introduce the wrong product identification into a process designed to rely on automated verification.
The result is a contradiction: pharmacies invest in sophisticated technology to reduce risk and improve efficiency, but inconsistent medication barcodes can prevent that technology from working as intended.
When a barcode fails, the pharmacy team still has to dispense the medicine and that means someone has to intervene.
Product details may need to be entered manually. Packaging may need to be checked. Alternative identification methods may have to be used. Temporary labels or other workarounds may be required. Every one of these interventions consumes staff time.
More importantly, it moves part of the dispensing process away from the automated workflow and back towards manual checking.
Our previous work on medication barcode accuracy highlighted this issue: when a barcode is missing or inaccurate, a crucial digital safety check can be bypassed and pharmacy teams can be forced to revert to manual verification.
That matters because manual workarounds don't just reduce efficiency, they can also introduce additional opportunities for human error. The irony is that the technology was introduced partly to reduce that risk in the first place.
It is tempting to view a barcode as simply a piece of information printed on a medicine pack. In a modern pharmacy, it is much more than that.
The barcode can form part of the chain of digital checks that connects the prescription, the dispensing system, the physical medicine and ultimately the patient. An accurate scan can help confirm that the product selected corresponds with what the system expects.
If the barcode is wrong or missing, that layer of verification is weakened. This is why barcode quality should be considered alongside other patient-safety controls, rather than being treated solely as a logistics or stock-management concern.
Patient safety is the most important consideration, but the operational consequences should not be underestimated.
Pharmacy teams are under continual pressure to deliver more services while managing increasingly complex workflows. Automation is intended to help by reducing repetitive tasks, improving throughput and allowing skilled pharmacy professionals to spend more time where they add the greatest value.
Barcode failures work in the opposite direction. Each manual intervention adds friction to the process. A single barcode problem may appear insignificant, but repeated across thousands of medicines and multiple dispensing locations, these interruptions can become a substantial workload.
The new GS1 UK findings demonstrate that this is not a theoretical problem. More than half of surveyed organisations reported moderate to severe disruption associated with barcode quality, while the impact was particularly pronounced among organisations using dispensing robots.
In other words, the more a pharmacy invests in automation, the more important reliable barcode data becomes.
The UK pharmacy sector is investing heavily in digital transformation. Barcode standards therefore need to be viewed as part of the infrastructure that makes this transformation possible.
If barcode quality is inconsistent, interoperability becomes harder, automated processes are interrupted and safety checks may be bypassed. There is understandably significant attention on medicines shortages, supply-chain resilience and improving visibility of medicines across the UK. Accurate barcodes can support all of those objectives. But their value extends much further.

The barcode may be small, but its role in modern pharmacy is anything but.
Technology has enormous potential to improve pharmacy dispensing. But technology cannot deliver its full value without reliable data at its foundation.
An automated dispensing system can be exceptionally sophisticated, but if the medicine it is trying to identify has no barcode, an unscannable barcode or an incorrect barcode, the system's ability to protect the patient and streamline the workflow is compromised.
That is why accurate medication barcodes should be treated as a patient-safety requirement, not simply a supply-chain preference.
Every successful scan represents more than a stock transaction. It is another digital confirmation that helps connect the medicine selected with the medicine prescribed and, ultimately, with the patient who is relying on it.
As pharmacy becomes increasingly automated, getting the barcode right is one of the most basic, and most important, things we can do to ensure that technology delivers on its promise.
Because the future of pharmacy dispensing may be digital, but patient safety still depends on getting the fundamentals right.
1. Audit your barcode scans - how often do your teams override or manually verify because of scan failures?
